Job summary

Health Watch Provo is seeking a compassionate RN to provide on-call coverage for Home Health and Hospice patients across Utah County and Salt Lake County. The RN will handle after-hours assessment, triage, and care coordination to ensure quality patient outcomes.

The ideal candidate has strong clinical assessment skills, effective communication, and the ability to work independently while collaborating with physicians and the interdisciplinary team.

Qualifications

• Current, active Utah RN license.
• Home Health/Hospice experience preferred.
• Excellent assessment and clinical decision-making skills.
• Strong communication and interpersonal skills.
• Ability to work independently and manage urgent situations.

Responsibilities

• Provide on-call nursing coverage for one designated week each month, including evenings, weekends, and holidays.
• Respond promptly to patient, caregiver, and staff calls regarding changes in condition and care needs.
• Perform telephone triage and determine appropriate interventions.
• Conduct skilled nursing visits when clinically indicated.
• Coordinate care with physicians, case managers, therapists, hospice staff, and other healthcare professionals.
• Assess, implement, and evaluate individualized patient care plans.
• Administer medications and monitor for effectiveness and adverse reactions.
• Educate patients and caregivers on disease processes, medications, safety, and resources.
• Document assessments and changes in patient condition accurately and timely.
• Report significant findings to the appropriate provider and supervisor.
• Maintain compliance with agency policies and PARTICIPATION requirements.
