Job summary

The Care Team Hospice seeks compassionate On-Call Registered Nurses (RNs) to provide after-hours and weekend support to hospice patients and families. Flexible weekday, weekend, and 7 On / 7 Off scheduling options are available, with independence and support from an experienced clinical team.

RNs respond to patient needs in the field or from home, focusing on symptom management and timely visits, while maintaining detailed EMR documentation and collaboration with the care team.

Qualifications

• Active and unrestricted Registered Nurse (RN) license in the state of practice.
• Minimum of one year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred.
• Strong clinical assessment, critical-thinking, and communication skills.
• Ability to work independently and confidently respond to changing patient needs.

Responsibilities

• Respond to after-hours patient and family needs, providing nursing assessments, symptom management, and clinical interventions.
• Complete scheduled and urgent patient visits, addressing changes in condition and providing compassionate end-of-life care.
• Educate and support patients and caregivers through symptom management, medication needs, and end-of-life transitions.
• Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care.
• Complete accurate and timely clinical documentation within the electronic medical record (EMR).
