Job summary

Christian Living Communities | Cappella Living Solutions seeks a Rehabilitation Flex Team Member (Occupational Therapy Assistant) to provide skilled nursing, assisted living and rehabilitation services to residents on a flexible, as-needed basis.

You will work under a licensed occupational therapist, implementing treatment plans and teaching home-based activities to residents and families.

Qualifications

• Education: Graduate of an accredited therapy program.
• Experience: 2–3 years of related experience, or an equivalent combination of education and experience.
• Licenses/Certifications: Current Occupational Therapy Assistant state license; CPR certification.
• Excellent verbal & written communication, active listening, and social/cultural perceptiveness.

Responsibilities

• Provides services under the direction of a licensed occupational therapist, collaborating with the care team.
• Follows treatment plans and teaches techniques for moving from bed to wheelchair and stretching.
• Assesses residents’ living skills and goals beyond diagnoses to tailor care.
• Helps develop and adjust activities and treatment plans to support independence.
• Builds trust-based relationships with residents and families in a warm, welcoming environment.
• Monitors and documents patient progress, and suggests changes to plans as needed.
• Educates residents and families on in-home programs and use of adaptive equipment.
• Uses equipment to safely move residents and performs routine care tasks.
• Documents care services per facility procedures.
• Collaborates with team to foster an inclusive workplace culture.
