Qualifications

• Must be licensed and/or registered as an OT.
• NBCOT-OTR designation (if state required).
• Current state OT license to practice.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Create comprehensive treatment plans with goals, frequency and duration.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver treatment using modalities and adjust as needed within precautions.
