Job summary

Thunder Bay Community Health Service is seeking a Nurse Practitioner or Physician Assistant to join as a Float Primary Care Provider. You will deliver comprehensive, patient-centered care to patients of all ages and rotate across multiple clinic locations to support teams.

Responsibilities include assessment, diagnosis, treatment, ordering tests, and coordinating care with physicians. BLS is required; license in Michigan and DEA registration are expected.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Michigan NP or PA-C license and DEA registration.
• BLS certification at hire is required.
• Team-oriented provider comfortable working across multiple clinic locations.

Responsibilities

• Provide comprehensive primary care to patients of all ages.
• Float between clinic locations to support providers and care teams as needed.
• Assess, diagnose, treat, and help patients manage overall health and wellness.
• Order and interpret diagnostic tests and develop individualized treatment plans.
• Collaborate with physicians and other healthcare professionals to ensure high-quality care.
