Bluestone Physician Services in Minnesota is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver on-site, team-based primary care within Assisted Living and Memory Care communities in and around Southwest Metro Minneapolis. This float-provider role supports patients with preventive, proactive care and care plans tailored to individual needs.

You will manage a patient panel of about 200, with a Monday-Thursday schedule and an optional Friday for clinical or administrative

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and /or maintain DEA certification for the State of required of work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required.

Responsabilités

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities in the capacity of a float provider.
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.).
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with community partners within home care, hospice, and other resources as appropriate.
