Mentor Community Services, part of the Sevita family, seeks a Regional Licensed Practical Nurse/Licensed Vocational Nurse/Nurse Case Manager (LPN) for a flexible daytime schedule in Pittston, PA. This role offers a meaningful nursing path with diverse responsibilities and no on-call rotation beyond expected coverage.

The position emphasizes nursing services, training, and supports for individuals with complex medical or behavioral needs.

Formación

• Associates degree in nursing or state certificate.
• One year experience with the population served in a nursing capacity.
• Current LPN/LVN licensure for state of residence.

Responsabilidades

• Provides prescribed medical treatment and personal care services.
• Performs health assessments and inputs health diagnosis for care plans.
• Monitors and records medications; liaises with pharmacy and administers medications.
• Oversees staff who administer medications and conducts QA checks on records.
• Provides emergency coverage and/or care as needed.
• Maintains confidentiality and follows Sevita policies and universal precautions.
• Works with external agencies to coordinate services and ensure consistent treatment.
• Maintains logs for staff and individuals' testing and trains staff in medication administration and infection control.
• May assist with household activities and transportation; participates in on-call system.
