Job summary

Mentor Community Services, part of the Sevita family, is seeking a Licensed Practical Nurse to provide nursing services and supports to individuals with disabilities in a community-based setting. You will address medical and psychiatric needs, administer medications, and collaborate with staff and external providers to coordinate care.

Qualifications

• Requires an Associate’s Degree in Nursing or related field, or state nursing certificate.
• Minimum 1 year of nursing experience with population served.
• Current state LPN/LVN license for residence.
• Valid driver’s license, car registration and auto insurance.
• State licensing certifications such as CPR/FA, HIV/AIDS, BBP, ACT training required.

Responsibilities

• Monitor health conditions and ensure quality services.
• Administer medications and supervise medication administration by staff.
• Provide prescribed medical treatment per physician orders and personal care.
• Conduct health assessments and update ISP-based health care services.
