Supportive Concepts for Families is seeking compassionate Licensed Practical Nurses (LPNs) to serve as Community Health Nurses, delivering ongoing clinical support and care coordination for adults with IDD living in community homes across Pennsylvania.

This field-based role offers autonomy, collaboration with RNs and physicians, and the opportunity to build lasting relationships while making a meaningful difference in health, wellness, and quality of life.

Qualifications

• Active Pennsylvania LPN license in good standing is required.
• Valid driver's license and reliable transportation are required.
• Strong clinical assessment, communication, and organizational skills are essential.
• Ability to work independently while collaborating with an interdisciplinary healthcare team.

Responsabilités

• Manage a caseload of individuals with routine medical needs.
• Provide ongoing nursing oversight and wellness monitoring.
• Administer medications and treatments per physician orders and scope of practice.
• Coordinate medical appointments and communicate with physicians, pharmacies, specialists, and families.
• Monitor chronic health conditions and report changes to RNs and providers.
• Review physician orders, med changes, labs, and clinical documentation.
• Educate Direct Support Professionals on med administration and health monitoring.
• Participate in interdisciplinary team meetings (IDT).
• Maintain accurate documentation and ensure compliance with standards.
• Collaborate with Community Health RNs to support higher-acuity individuals.
