Rehab Without Walls Neuro Rehabilitation in the United States seeks an experienced LCSW to provide counseling across physical, cognitive, and emotional recovery in home and community settings.

You will perform comprehensive evaluations, set team-based objectives, support health professionals in emotional factors, counsel patients and families, coordinate care, and connect patients with community resources to meet measurable goals.

Formación

• Master's Degree in Clinical Social Work (LCSW).
• Current LCSW licensure in the state of practice.
• At least one year of rehab setting experience (TBI/SCI/acute).
• BLS First Aid and CPR certification.
• Ability to lift 50 pounds (up to 100 pounds with safety procedures).
• Physical requirements include lifting, moving, standing for extended periods.

Responsabilidades

• Performs comprehensive evaluations of social, emotional and support needs.
• Sets measurable objectives with the rehab team.
• Assists health team in understanding social and emotional factors.
• Instructs/counsels patients and families on coping with illness.
• Participates in care coordination and resource identification.
• Identifies community resources to reach measurable objectives.
