Rehab Without Walls Neuro Rehabilitation in Hollywood, FL, seeks a PRN Licensed Clinical Social Worker (LCSW) to join the Home & Community Neuro Rehabilitation program. You will assess patients, provide emotional support, and connect families with community resources to promote independence and better quality of life.

The role offers a flexible PRN schedule, collaboration with an interdisciplinary team, and opportunities for professional development within a national neuro-rehabilitation network.

Qualifications

• Master’s Degree in Clinical Social Work (LCSW) required.
• Current LCSW licensure in the state of practice.
• At least 1 year of rehab experience with brain/spinal injuries or acute diagnoses.
• BLS and CPR certification required.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Physical requirements include standing/walking for extended periods.

Responsabilités

• Provide psychosocial assessments and emotional support for patients and families.
• Assist with coping strategies, adjustment, and community reintegration.
• Connect patients with community resources and support services.
• Collaborate with therapists, physicians, case managers, and caregivers to support treatment goals.
• Advocate for patients throughout their rehabilitation journey.
• Perform comprehensive evaluations of social and emotional needs.
• Set measurable objectives with the rehab team.
• Support care coordination activities and resource identification.
