FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages in Thermal and surrounding areas. This Independent Contractor role serves patients in their homes, providing compassionate, patient-centered care.

You will control your schedule and coverage areas, with strong per-visit compensation potential. FeldCare Connects offers administrative support, streamlined referrals via Cliniconnects, and a focus on one-on-one

Competenze

• Must hold an active Physical Therapist license issued by the state board.
• Completion of an accredited Physical Therapy program is required.

Mansioni

• Deliver one-on-one PT services in patients' homes.
• Observe patient response and adjust treatment plan accordingly.
• Document care and progress in the EMR system and coordinate with FeldCare Connects team.
