Job summary

Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Elyria, OH. The role offers a flexible schedule and the opportunity to work outside a traditional clinic setting.

Practitioners will travel to patients' homes, conduct comprehensive, non-invasive assessments, review histories, and provide basic education while receiving training and scheduling support. A valid NP license and board certification are required.

Qualifications

• Masters Degree OR commensurate experience and NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
