Job summary

Source One Management Services, LLC in Putnam, CT seeks Nurse Practitioners and Physician Assistants for flexible in-home Health Risk Assessments for Medicare Advantage members. Work autonomously as a 1099 contractor with an 8-day monthly minimum and no on-call duties.

You will review medical history, meds, lifestyle and social factors, perform basic health evaluations, and travel locally within about an hour from home.

Qualifications

• Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health)
• Minimum availability of eight full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of home

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review medical history, current medications, lifestyle, and social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
