Job summary

Source One Management Services, LLC in Westerly, RI seeks Nurse Practitioners and Physician Assistants for flexible PRN in-home Health Risk Assessments for Medicare Advantage members. You’ll work autonomously and schedule eight 7.5-hour days per month with no prescribing or on-call duties.

You’ll complete HRAs, review medical history, meds, lifestyle, and social factors, perform basic physical and mental health evaluations, and travel locally to patients within about an hour of home.

Qualifications

• Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health)
• Minimum availability of eight (8) full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of your home

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review patient medical history, current medications, lifestyle, and key social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
