Job summary

Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This PRN role offers autonomy, a company-provided iPad for documentation, and malpractice insurance.

Responsibilities include completing HRAs, reviewing medical histories, and performing basic health evaluations with travel to local patients within ~60 minutes of your home.

Qualifications

• Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health specialties)
• Minimum eight (8) full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of your home

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review patient’s medical history, current medications, lifestyle, and key social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
