Job summary

Qualifications

• Active NP or PA license.
• Minimum availability of eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review patient history, medications, lifestyle, and social factors.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
