Qualifications

• Active NP or PA license required (excluding Pediatrics, PMHNP, Women’s Health).
• Availability: eight 7.5-hour days per month minimum.
• Willingness to travel within the local area.

Responsibilities

• Perform comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
• Review medical history, medications, lifestyle, and social determinants.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
