Job summary

Achieve Behavior Alliance Plus is seeking HCSP Service Opportunities roles—Independent Contractors providing PAC, respite, day habilitation, and other support in Fortville, IN. Flexible evenings and weekends, in-home/community-based care.

Contractors will work with HCSP Coordinator, choose client load, schedule, and services. Role emphasizes person-centered support, daily living assistance, documentation, and collaboration with families.

Qualifications

• Must be 18 years of age or older.
• High school diploma or equivalent preferred, not required.
• Valid driver’s license with reliable transportation to and from clients location.
• Clean background check.
• Ability to obtain required certifications at start of contract.

Responsibilities

• Provide person-centered, direct care support to individuals with IDD.
• Assist with daily living activities including meal prep and hygiene.
• Follow each individual’s support plan to promote skill development.
• Maintain a safe, respectful environment while protecting dignity and privacy.
• Observe and report changes in behavior, health, or well-being.
• Complete accurate service documentation and session notes.
• Communicate professionally with families and coordinators.
• Coordinate schedules directly with individuals and families.
• Supervise and engage individuals during caregiver absences.
• Facilitate social, recreational, and skill-building activities.
