Qualifications

• Licensed in and living in North Carolina.
• MD or DO License (Preferred).
• Doctorate (Required) to practice as a physician collaborator.

Responsibilities

• Perform in-home health evaluations for health plan members.
• Review medication history and conduct a physical evaluation in the member's home.
• Do not prescribe medicines, order labs, or alter current treatment regimens.
• Collaborate with Signify Health-contracted nurse practitioners and physician assistants as assigned.
