Job summary

Signify Health, Inc. seeks physicians to conduct in-home health evaluations for health plan members. Qualifications

• Licensed to practice medicine in North Carolina.
• MD or DO license (preferred).
• NC residency and license required to qualify for home-visit roles.

Responsibilities

• Oversee Signify Health-contracted nurse practitioners and/or physician assistants.
• Perform in-home health evaluations for health plan members.
• Review medications and medical history; perform physical evaluations in patients' homes.
• Provide a pre-populated form for each evaluation; no prescribing or testing orders.
