Job summary

Signify Health, Inc. offers a flexible physician collaboration role with a self-determined schedule and the potential to increase earnings through a pay-per-test model.

Physicians oversee Signify Health-contracted nurse practitioners and/or physician assistants to support in-home evaluations. Responsibilities include conducting in-home evaluations, reviewing medical history, and coordinating care with health plan members.

Qualifications

• Active medical license in North Carolina.
• Experience with home health evaluations preferred.
• Ability to work independently with a flexible schedule.
• MD/DO license preferred for collaboration.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Oversee Signify Health-contracted NPs/PAs to coordinate patient care.
• Utilize pre-populated forms for assessments and maintain accurate documentation.
