Job summary

TriSearch is seeking a Nurse Practitioner to provide expert care across home, facility, and clinical settings in Connecticut. The position supports screenings, assessments, and management of patient needs with flexibility for PRN and full-time schedules.

The role requires RN/NP licensure in CT, board certification, and prescriptive authority where applicable. Travel to scheduled locations and collaboration with diverse care teams are expected.

Qualifications

• Must have active RN and NP licensure in state of practice including prescriptive authority.
• Master’s Degree in Nursing or equivalent and NP licensure.
• Board certification in Adult/Family/Geriatrics Primary Care or ACNPC-AG/ACNPC.
• Current CPR certification and eligible for multistate licensure.

Responsibilities

• Perform screenings including medical history, physical exam, medication reconciliation and education.
• Identify, evaluate and address concerns of member or participant.
• Order and interpret diagnostics and provide interventions or referrals as needed.
• Administer and order point-of-care and lab-based testing.
• Refer members to PCP, case management, or specialists as necessary.
• Collaborate with risk, quality, staffing, physicians and care teams.
