Job summary

Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around New Britain, CT. This role offers flexibility to align with your personal goals while delivering essential care in a community setting.

Nurse Practitioners will travel to patients' homes, conduct non-invasive assessments, review histories, and document findings. Training is provided, with liability insurance options and scheduling support to facilitate patient care.

Qualifications

• Master's degree in Nursing or commensurate NP experience.
• Active NP license in CT.
• National board certification required.
• Current BLS certification.
• 12 months of paid clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.
