Nest & Care is seeking a dedicated PRN Registered Nurse to join our home care team, delivering essential nursing care to patients in their homes. The role offers flexible scheduling and the chance to make a meaningful difference for patients and families.

Key duties include client assessments, care planning, care coordination with the team, and clinical oversight. Strong trauma and trach experience with MD RN licensure is required, along with travel to cases as assigned.

Qualifikationen

• RN state license (MD) with Trach care experience is required.
• CPR and First Aid certification.
• Clear criminal background and driving record.
• Two professional healthcare references.

Aufgaben

• Meets new and prospective clients and performs initial assessments (virtual or in-person).
• Develops a Plan of Care for new clients and updates as needed.
• Communicates with care managers, caregivers, office staff, clients, and families about needs and concerns.
• Provides supervision with Clinical Director regarding services.
• Trains clients and staff on care procedures and documentation.
• Maintains clinical records compliant with state and federal laws.
• Performs periodic client evaluations and caregiver evaluations as required.
