Job summary

Hueman seeks a qualified Nurse Practitioner to conduct in-home health assessments and engage patients in a more personal, preventive care approach. NPs in this role travel to patients' homes and perform comprehensive, non-invasive assessments while reviewing medical histories and documenting findings.

Successful candidates hold an NP license, board certification, BLS, and prefer 12 months of paid clinical provider experience.

Qualifications

• Master's degree in nursing or commensurate experience with NP license.
• Active NP license in the state.
• National board certification as NP.
• Current BLS certification required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventive care.
