Job summary

TriSearch is seeking a Nurse Practitioner (NP) to provide expert care, including clinical assessments and screenings, in diverse settings such as member homes, assisted living, and skilled nursing facilities.

This role offers both PRN and full-time opportunities; requires NP licensure in NY, board certification, and ability to travel; strong patient communication and documentation skills are essential.

Qualifications

• Bachelor of Science in Nursing preferred.
• Master’s Degree required OR commensurate experience and NP licensure.
• Current RN and NP licensure with prescriptive authority or ability to obtain.
• Board certification by AANP, AACN or ANCC in specified Adult/Family/Geriatrics areas.
• Current CPR certification.
• Multistate licensure preferred.

Responsibilities

• Perform screenings including medical history, physical exams, medication reconciliation and education.
• Identify, evaluate, and address member concerns.
• Order and interpret diagnostic imaging and labs; provide interventions or referrals.
• Administer and order point-of-care and lab-based testing.
• Refer members to PCP, case management, disease management programs, or specialists.
• Collaborate with risk, quality, staffing, physicians, case managers, social workers and care teams.
• Participate in mandatory compliance and training; adhere to laws and Matrix policies.
• Create and maintain documentation per clinical standards and protocols.
• Maintain PHI and comply with OSHA and State Board of Nursing.
• May act as a preceptor for new providers.
