Job summary

Northwestern Medicine is seeking a Registered Nurse - Flex HHC to provide direct patient care across the care continuum, from admission to discharge. The role emphasizes evidence-based practices and collaboration with the healthcare team to develop and implement individualized care plans.

The nurse will assess patients, coordinate care, document progress, and participate in discharge planning while maintaining compliance with ethics and compliance programs.

Qualifications

• BSN degree required.
• Current Illinois Registered Nurse license.
• American Heart Association BLS certification.
• Strong interpersonal and collaboration skills.
• Strong analytic abilities.
• Maintains valid driver’s license and auto insurance.
• Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN within three years of first day in clinical nurse role.

Responsibilities

• Assess and record patient status and needs at admission/point of care entry.
• Organize health data so it is accurate, complete and accessible.
• Identify problems needing immediate attention and coordinate interdisciplinary care.
• Perform joint home visits with Charge Nurse and validate diagnoses with patient/family and health care team.
• Document diagnoses and plan of care; initiate discharge planning at admission.
• Provide patient education and plan care with measurable outcomes; reassess and update Plan of Care.
• Communicate with patient/family and health care team re: care and progress; present information at multidisciplinary meetings.
• Make referrals for continuity of care and ensure evidence-based nursing interventions are followed.
