Job summary

Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA) in Bay County and surrounding areas, offering a flexible schedule and the autonomy to provide one-on-one in-home care. You will deliver hands-on therapy following a PT plan of care, helping patients improve mobility and independence, and collaborating with families and care teams.

Compensation is $45-$50 per visit based on experience, with no hospital shifts and AI-assisted charting to save time.

Qualifications

• Active Florida PTA license required.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.

Responsibilities

• Provide hands-on therapeutic care per the plan of care.
• Help patients improve mobility, strength and independence.
• Collaborate with patients, caregivers, and interisciplinary teams.
• Document progress using AI-assisted tools.
