Job summary

Interim HealthCare is seeking a dedicated Physical Therapist (PT) for home health in the City of Pittsburgh area. You will evaluate and treat patients in their homes, establishing and guiding individualized treatment plans to improve mobility and reduce pain.

Qualifications

• Current, unrestricted license to practice as a Physical Therapist (PT) in the state of employment.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites.
• At least 1 year of PT experience in home health or similar settings.

Responsibilities

• Evaluate and treat patients with physical disabilities or reduced mobility in a home health setting.
• Develop and administer individualized treatment plans using exercises, stretching, manual therapy, and equipment.
• Educate patients and families on rehabilitative care and activities to promote health and independence.
• Collaborate with other providers to achieve the best patient outcomes.

Education
