Job summary

Interim HealthCare Inc. is seeking a Physical Therapist (PT) for full-time home health care. You will evaluate and treat patients with mobility challenges in their homes, establishing treatment plans and educating patients and families to support independence and safety.

The role emphasizes one-on-one patient care, collaboration with a care team, and delivering compassionate, high-quality service. licensure and CPR/BLS certifications are required, with transportation to care sites necessary.

Qualifications

• Current unrestricted Physical Therapist license in the state of practice.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• At least 1 year of professional PT experience in home health or similar setting.

Responsibilities

• Evaluate and treat patients in a home care environment for mobility issues.
• Develop and administer a treatment plan using exercises, manual therapy, and equipment.
• Educate patients and families to promote health, safety and independence.
• Collaborate with other providers to achieve optimal patient outcomes.
