Job summary

Concierge Home Care is hiring a Home Health Occupational Therapist (OT) to provide 1:1 in-home therapy across St Augustine and St Johns County. Earn an annual salary of $110,000+ with potential for top performers to reach $175K+ and quarterly bonuses.

Enjoy flexible scheduling, autonomy, and mileage/data plan stipends in a role focused on meaningful patient relationships. Evaluate, treat, and educate patients to improve ADLs and mobility, while collaborating with physicians and the care team.

Qualifications

• Active Florida Occupational Therapist license.
• CPR certification.
• Valid driver's license and reliable transportation.
• Ability to supervise COTAs.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized home health treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs and functional mobility.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
