Job summary

Cleveland Clinic's Center for Connected Care seeks an Associate Staff physician to join our growing home-based primary care practice serving older adults in their homes on the west side of our territory. The role blends technology-enabled care with compassionate, patient-centered service and may involve in-home visits, geriatric assessments, and supervision of NPs.

This flexible opportunity supports office-based time, SNF, virtual medicine, or inpatient care as needed, with partial weekend call.

Qualifications

• Must be board-certified or board-eligible in Internal Medicine or Family Medicine.
• Strong interest in geriatrics preferred; training available.
• Experience with home-based primary care is a plus.

Responsibilities

• Provide in-home primary care, geriatric assessment and consultation.
• Supervise advanced practice nurses and provide medical oversight of home care and hospice services.
• Engage in leadership, research, program development and medical education as needed.
• Participate in weekend call, primarily telephone/home call, with other team members.

Education
