Responsabilités

• Coordinate referrals, lab orders, and specialist appointments.
• Counsel patients on discharge meds to reduce readmissions.
• Provide bedside care reflecting Paramount's standards.
• Communicate clearly with patients, families, and care teams.
• Handoffs at shift changes using SBAR with no gaps.
• Support families during prognosis discussions and code status talks.
• Maintain a fast-paced, accurate charting discipline in ER.
