OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Reading, PA to provide hands-on, person-centered support for individuals with IDD in home and community settings. You will implement care plans, assist with daily living activities, and promote independence while adhering to HIPAA and regulatory standards.

The role requires strong documentation skills, use of EVV systems, and flexibility, including evenings or weekends as needed.

Qualifications

• High school diploma or equivalent is required.
• Experience in caregiving, home health, or direct support for IDD is preferred.
• English proficiency required; bilingual skills are a plus depending on program needs.

Responsabilités

• Provide hands-on, person-centered support according to care plans.
• Assist with daily living activities, meals, mobility, and health monitoring.
• Support community participation and transportation to appointments or outings.
• Maintain accurate documentation and follow ISP/IPP guidelines.
