Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Philadelphia, PA. The DSP provides hands-on, person-centered support to individuals with IDD in home and community settings, promoting independence and well-being.

Responsibilities include assisting with daily living, nutrition, mobility, medication support, behavior strategies, and ensuring safety. Flexible schedule; field-based travel to homes and community sites is required.

Qualifications

• English proficiency required (spoken and written).
• Ability to use electronic documentation systems and EVV.
• Basic computer skills including email and mobile apps.
• Follow written care plans and documentation guidelines.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety and emotional well‑being.
• Promote independence, stability, and community participation; maintain structured routines.
• Support employment or vocational activities and transportation to appointments or outings.
• Participate in trainings and team meetings; maintain accurate documentation.
