Job summary

Devereux Advanced Behavioral Health seeks a Blended Case Manager to coordinate care across mental health, medical, and educational services, and to connect families with local resources in Chester County, PA. Primary duties include maintaining contact with clients, developing care plans, and ensuring timely documentation.

Qualifications

• Bachelor's Degree in Human Services field (Psych, Social Work, Sociology, Criminal Justice, etc…).
• 2 years’ experience in public or private human services with 1 year in direct client contact.

Responsibilities

• Coordinate care of individuals in mental health, medical, educational, placement and related areas. Assess needs using identified tools to develop initial plan of care.
• Connect families to needed resources in their local community - which could include educational, financial, mental health, physical health, housing, food insecurity, and other resources.
• Coordinate monitor and review the provision of services to ensure individuals are receiving and participating in services on a continual basis.
• Maintain direct contact (telephone and/or face to face) with all individuals involved in assigned caseload per state regulations.
• Ensure the timely and accurate completion of related documentation to include service documentation, outcome reports, service plans, face sheets, FPI, authorizations, releases and assessments.
