Job summary

PruittHealth Hospice in Cumming, GA seeks a FLEX REGISTERED NURSE CASE MANAGER (RN) to complete admissions, peaceful path visits, and routine hospice visits for patients in the home or facility setting.

This full-time, 1st shift role offers top-of-market pay, annual merit increases, paid time off, mileage reimbursement, a preceptorship program with hands-on training, and comprehensive health plans under a supportive company culture.

Qualifications

• Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
