Job Description Summary
The CARE Case Manager coordinates routine and crisis casemanagement for the WFU students, faculty, staff, and community.This position serves as a member of the WFU CARE Team. Additionalresponsibilities include community education and outreach, datacollection, relevant special projects, and other general casemanagement duties for the Office of the Dean of Students incollaboration with University and community partners.

Essential Functions:
- Provide case management services for students, staff, andfaculty experiencing personal, medical, or mental health-relatedcrises. This includes helping individuals become connected with onand off-campus support resources, communicating appropriately withall parties involved, and engaging in timely documentation.
- Serve as one primary point of contact for the WFU communityregarding. persons in need of caring outreach and/or casemanagement coordination).
- Maintain case notes containing sensitive information andprovide accurate and thorough documentation of weekly Teammeetings, emergency meetings, as well as daily case managementactivities throughout the week via electronic record-keeping.
- Provide support for CARE Team activities (e.g., weekly and adhoc meetings).
- Collaborate with campus partners, departments, schools, andcolleges to achieve case management goals.
Required Education, Knowledge, Skills, Abilities:
- Master’s degree in Social Work, Mental Health, Human Services,Higher Education, Counseling Divinity, or related field and twoyears of relevant work experience.
- Excellent interpersonal communication skills; ability tocommunicate effectively with groups and work and communicate wellin a team environment.
- Ability to assess information regarding identified needs forcase management, caring outreach, and persons of concern.
- Ability to professionally and appropriately manage sensitive,private material and information.
- Knowledge, experience, and comfort with recognizing andsupporting individuals experiencing mental health issues andsymptoms.
- Knowledge of risk factors and warning signs related to thepotential for targeted violence on campus and within thecommunity.
- Ability to synthesize and summarize large amounts ofinformation, document information electronically in a timelymanner, and verbally communicate with appropriate stakeholders aswarranted.
- In times of crisis intervention, work may require night andweekend availability.
- Knowledge of WFU and community resources, ability tocommunicate effectively to facilitate referral to thoseresources.
- Ability to tolerate, convey, and protect sensitiveinformation.
- Understanding of and ability to adhere to strict privacyrestrictions.
- Skill in using technology to track and manage individual casesas well as assess and report on campus trends.
- Knowledge of current regulations relevant to work in highereducation (i.e., Title II, III, & IX; FERPA; HIPAA;etc.).
Preferred Education, Knowledge, Skills, Abilities:
- Three to five years of relevant work experience.
- Experience with case management in organizations, specificallyin higher education.
Accountabilities:
- Responsible for own work.
Physical Requirements:
- Sedentary work that primarily involves sitting/standing.
- Occasionally moving objects up to 20 pounds.
Environmental Conditions:
