Job summary

Wake Forest University is seeking a CARE Case Manager to coordinate routine and crisis case management for the university community in Winston-Salem, NC. Qualifications

  • Master’s degree in Social Work, Mental Health, Human Services, Higher Education, Counseling or related field.
  • Two years of relevant work experience.
  • Excellent interpersonal communication skills; ability to communicate effectively with groups and work in a team.
  • Ability to assess information for case management, outreach, and referral needs.
  • Ability to maintain sensitive information with privacy and confidentiality.
  • Knowledge of mental health issues, risk factors, and campus resources.

Responsibilities

  • Provide case management services for students, staff, and faculty in personal, medical, or mental health crises.
  • Serve as a primary point of contact for care outreach and case management coordination.
  • Maintain case notes and document weekly team meetings and daily activities in electronic records.
  • Support CARE Team activities and participate in meetings.
  • Collaborate with campus partners to achieve case management goals.
