Job summary

Betty Hardwick Center in Abilene, TX seeks a Care Coordinator for STEP, coordinating intensive services for adults with early psychosis. You will support goal setting, coping skills, and recovery, while linking clients to community resources.

The role includes outreach and ensuring thorough documentation. Requirements include a Bachelor's (Master's preferred) in social/human services, QMHP CS certification, and a valid Texas driver’s license.

Qualifications

• Bachelor's degree in social, behavioral or human services; a Master's degree is preferred.
• Must be able to be certified as a QMHP CS.
• Experience in mental health/intellectual and developmental disabilities services preferred.
• Experience in supported employment and/or supported housing services preferred.
• Must have a valid Texas Driver's License and be insurable to drive as part of the role.

Responsibilities

• Provide coordination, monitoring and rehabilitative services for adults meeting FEP eligibility.
• Assist eligible individuals to clarify goals, cope with stress, and overcome barriers to recovery.
• Engage in outreach and recruitment activities.
• Develop and maintain professional relationships with service entities and document activities.
