Job summary

Southwest Key Programs is seeking a Youth Tracker to perform daily youth contact and ensure service plan adherence. The role requires flexibility to work hours beyond the regular schedule, including evenings, weekends, and holidays.

Responsibilities include maintaining youth records, utilizing JTS and ETO, and responding promptly to crises. Travel across GA counties is expected, with staff using personal vehicles and IRS mileage reimbursement provided.

Qualifications

• High School Diploma or GED is required.
• Must be able to conduct daily contacts with youth.
• Strong communication and writing skills are essential.

Responsibilities

• Conduct daily face-to-face contacts with youth at various locations.
• Manage behaviors using programmatic and individual structure.
• Develop helpful relationships and promote youth accountability.
• Complete required documentation, including discharge information, accurately and timely.
• Enter data into JTS and ETO systems promptly.
• Review youth files and stay updated with new information.
• Notify supervisor during crises or serious incidents.
• Maintain regular and punctual attendance.
