Qualifications

• High school diploma or GED required.
• Valid driver’s license with acceptable driving record.
• Reliable personal transportation and ability to travel.
• Strong interpersonal and relationship-building skills.
• Excellent written and verbal communication skills.
• Ability to prepare accurate, objective documentation.
• Strong organizational, time-management, and follow-through skills.
• Ability to work independently with limited supervision.
• Ability to interact professionally with diverse backgrounds.
• Proficiency with Microsoft Office, smartphones, tablets and web-based applications.

Responsabilités

• Conduct scheduled field and community-based visits.
• Meet with individuals and families in residential and community settings.
• Gather, verify, and document information per program protocols.
• Observe and document health, safety, and welfare issues.
• Report potential concerns per procedures.
• Maintain professional boundaries with empathy and cultural awareness.
• Prepare clear, timely case notes and field reports.
• Maintain confidential case documentation.
• Utilize mobile devices and electronic case-management systems.
• Travel to geographic areas for assignments as required.
