Qualifications

• Requires MS/MA in social work, counseling, or related behavioral health field or nursing.
• Minimum 3 years of clinical social work/counseling experience with complex psychiatric/substance abuse.
• Active, current license such as LCSW/LMSW/LPC/LAPC/LMFT/LMHC or RN in Virginia.

Responsibilities

• Conduct assessments to identify individual needs and develop care plans.
• Coordinate with internal/external resources for integrated, whole-person care.
• Assist with education, medication reconciliation, and follow-up care.
