Qualifications

Responsibilities

• Manage patient care and scheduling.
• Monitor, record and report symptoms and changes in patients' conditions.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patient responses.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
