Job summary

Allegiance HH West Central LA Traditional is seeking an RN to manage patient care, coordinate schedules, and monitor patient conditions. The role involves maintaining detailed patient records and collaborating with the healthcare team to ensure quality care in home health settings.

The position requires an Associate’s or Bachelor’s Degree in nursing, current LSBN license without restrictions, and 1+ year of nursing experience; home health exposure is preferred.

Qualifications

• Education: Associate’s or Bachelor’s Degree (four year college or university).
• RN license with LSBN without restrictions; one year of clinical nursing experience or waiver for LPN background.
• Two years preferred nursing experience and home health exposure.

Responsibilities

• Manage patient care and scheduling.
• Monitor, record and report symptoms and changes in patients' conditions.
• Maintain accurate, detailed reports and records.
• Order, interpret, and evaluate diagnostic tests to identify patient condition.
• Direct and coordinate infection control and nursing care plans.
• Instruct patients, families and groups on health education and disease prevention.
