Job summary

Qualifications

• RN licensed by LSBN with no restrictions.
• 1–2 years of related clinical experience.
• Associate’s or Bachelor’s Degree in Nursing is required.
• Proficiency with Microsoft Office, Google Suite, and WellSky preferred.

Responsibilities

• Manage patient care and scheduling.
• Monitor, record and report symptoms and changes in patients' conditions.
• Maintain accurate, detailed reports and records.
• Process, validate, and analysis of OASIS.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
• Educate individuals, families and other groups on health topics and disease prevention.
• Record patients' medical information and vital signs.
• Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
• Direct and supervise less skilled nursing or health care personnel.
