Job summary

Allegiance HH Northwest in Minden, LA seeks a Registered Nurse to manage patient care, schedules, and OASIS-related tasks. You will create 485 forms, adjust treatment plans as physicians order, and coordinate with the health care team to ensure high-quality patient outcomes.

The role requires a Louisiana RN license with at least one year of clinical experience and BLS. You will work in primary or remote offices with occasional patient home visits, contributing to education and infection-control

Qualifications

• Minimum 1 year of clinical RN experience.
• Valid Louisiana RN license without restrictions.
• Basic Life Support (BLS) required.

Responsibilities

• Manage patient care and scheduling.
• Maintain accurate, detailed reports and records.
• Process, validate, and analysis of OASIS.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
• Monitor all aspects of patient care, including POC being carried out as written and goals met.
• Instruct individuals, families and other groups on health education and disease prevention.
• Ensure the team is educated on needs of individuals, families or communities.
• Inform physician of patient's condition as required.
• Observe nurses and visit patients to ensure proper nursing care.
• Acts as liaison between doctor and staff.
• Direct and coordinate infection control programs and precautions.
• Recommend where appropriate drugs or therapies.
• Consult with institutions regarding nursing practice concerns.
• Engage in training or research activities related to nursing.
• Perform other duties as assigned by management.
