Job summary

Choice Health at Home is seeking a Home Health Field Registered Nurse (RN) for PRN coverage in the Lake County area, including Ocala, FL. The role emphasizes comprehensive in-home assessments, care planning, and multidisciplinary collaboration to support recovery and chronic disease management.

You will educate patients and families, coordinate with physicians, and ensure continuity of home-based care while maintaining safety and quality standards.

Qualifications

• Current RN license required.
• Valid driver’s license essential.
• CPR certification required.
• HomeCare HomeBase experience preferred.
• At least 2 years of Home Health RN experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs.
• Develop, implement and document the plan of care; revise as necessary.
• Educate patients and families on self-care techniques, nutrition, and dietary needs.
• Determine scope and frequency of services based on acuity and needs; assess caregiver readiness.
• Initiate preventive and rehabilitative nursing procedures.
• Provide clinical directions to Home Health Aide and LPN/LVN to ensure quality care.
• Prepare progress notes and report changes to physician and team.
• Monitor cases for third-party payor requirements.
