Job summary

Choice Health at Home in Ocala, FL is seeking a Home Health Field RN to deliver patient-centered care in patients' homes. The RN conducts assessments, plans, implements, and evaluates nursing care tailored to each patient, coordinating with families and interdisciplinary teams.

You will perform admissions criteria, develop plans of care, educate patients on self-care, and ensure continuity of care. This full-time role requires a valid RN license and a valid driver's license.

Qualifications

• Current RN License
• Valid Driver’s License
• CPR Certification
• Valid Auto Insurance
• At least 2 years of Home Health RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Perform initial and ongoing comprehensive assessments and treatment in patients' homes.
• Develop, implement, and document the plan of care and revise as needed.
• Educate patient and family on self-care techniques, nutrition, and medications.
• Determine scope and frequency of services based on acuity and needs.
• Provide clinical directions to Home Health Aide and LPN/LVN to ensure quality care.
• Prepare progress notes and inform physician of changes in patient status.
